The Science Education Department at Harvard University is inviting middle school physical science educators to collaborate with us in our study on improving multiple choice questions.

We are interested in the assessment questions and tests that middle school physical science educators use with their students. We invite you to collaborate with us by sharing your tests or assessment items that you use with your students, particularly multiple-choice questions. We ask you to submit a quiz or test that has at least 5 multiple choice questions along with the answer choices for these questions, for which we will send you a $50 Amazon gift card as our thanks for helping us.

Please complete this short survey telling us a bit about yourself, and then include your multiple-choice questions (at least 5 questions) and answer choices to them at the end of the survey. You can upload a clear image (pdf, word doc, photo) of a test or test questions/answer choices at the end of the survey form. Complete and submit the Amazon form to Cynthia Crockett at ccrockett@cfa.harvard.edu.
